This form is intended for fellows who have either lost their formal posted invite or who are interested in attending a meeting at short notice but
no later than the Friday before a Monday meeting. By filling out this form the details are emailed to Betty Smallwood and members are invited to give her a cheque for their attendance at the meeting itself. Please do not attempt to type in any financially sensitive information - the contact details are optional.
